- Annual Pap/Pelvic Exam. Patient must have had a Pap/Pelvic exam in the last three years and not given birth to a new baby since the last exam.
- Family Planning Problem
- Patient is receiving family planning method/birth control and is having a problem (change of method, other issues/problems)

- Patient needs supply of birth control method (pills, Depo, ring). Patient MUST have had a physical exam (Pap/Pelvic) from the Health Department in the last year.
