      • Adult Immunization (shots)- Patient has a voucher to receive vaccination (shots) through their employer or an organization.

  • Pediatrics

    • Pediatrics Well Child Exam (school physical)
      • Well Child exam (school physical).  Uninsured/Medicaid, no Commercial/Private Insurance except Anthem and Humana
    • Pediatrics-Immunizations (shots)
      • Pediatric Immunizations (shots)- Patient must bring a copy of all immunizations (shots) received from outside providers other than the health department.  Vaccines for children (VFC) only (uninsured, underinsured, Medicaid).  No Private/Commercial Insurance

  • Sexually Transmitted Disease (STD)

  • Tuberculosis (TB)
