    • Family Planning Exam- Initial
      • Patient has not had a Pap/Pelvic exam in the last three years or not given birth to a new baby since the last exam.
    • Family Planning Exam-Annual
      • Annual Pap/Pelvic Exam.  Patient must have had a Pap/Pelvic exam in the last three years and not given birth to a new baby since the last exam.
    • Family Planning Problem
      • Patient is receiving family planning method/birth control and is having a problem (change of method, other issues/problems)
    • Family Planning Pregnancy Testing
      • Pregnancy Testing
    • Family Planning Supply
      • Patient needs supply of birth control method (pills, Depo, ring).  Patient MUST have had a physical exam (Pap/Pelvic) from the Health Department in the last year.

    • Pediatrics Well Child Exam (school physical)
      • Well Child exam (school physical).  Uninsured/Medicaid, no Commercial/Private Insurance except Anthem and Humana
    • Pediatrics-Immunizations (shots)
      • Pediatric Immunizations (shots)- Patient must bring a copy of all immunizations (shots) received from outside providers other than the health department.  Vaccines for children (VFC) only (uninsured, underinsured, Medicaid).  No Private/Commercial Insurance
  • Sexually Transmitted Disease (STD)

  • Tuberculosis (TB)
